All new patients complete the new patient information form, plus the registration form for the type of care you are coming in for. Reception will confirm which applies to you.
Read as soon as your date is confirmed — some medication changes need to start up to a week before admission.
Separate from the forms above. Each hospital collects its own admission details, and this must be completed before your procedure — the rooms cannot do it on your behalf. You will need your procedure date and your health fund details to hand. Most hospitals use an online portal and publish a patient guide covering arrival, parking, your room and what to bring; Cremorne instead sends you a questionnaire link once your booking is made.
Register as soon as your date is confirmed. Hospitals commonly require admission details at least a week ahead, and an incomplete registration can delay or cancel your admission on the day.
Forms completed online are submitted over an encrypted connection straight into your medical record. Your answers are not stored on the website.
